Request for Iowa Residency Status Form

The determination for resident and non-resident status is based on the purpose of being in Iowa. I hereby state that I am currently in Iowa as a permanent resident and plan to work and live here. My primary reason for being in Iowa is for a reason other than to attend school. I have established a permanent residence in the State of Iowa and have resided in said residence for at least ninety (90) days prior to the start of the semester and year indicated below. In accordance, therefore, with proper procedures and submission of documented evidence of residency, I am requesting residency status classification for tuition purposes only. Granting this request does not in and of itself establish eligibility for Iowa financial aid programs.
