Drop Request Form

All changes take effect as of the date received and approved by Enrollment Services. Incomplete forms will not be processed. Proper registration is the responsibility of the student.

Please be aware of the last date to add and drop classes and the refund policy for each semester (see Important Dates for details) and the effect this may have on your billing and/or your financial aid.
