Add Request Form

**All changes take effect as of the date received and approved by the Enrollment Services. Incomplete forms will not be processed. Proper registration is the responsibility of the student.

Please be aware of the last date to add and drop classes (see Important Dates for details) and the effect this may have on your billing and/or your financial aid.

Enter course information and instructor email. Your request will be sent to the instructor for approval. It is the instructor's responsibility to send the approval to Student Records before you can be added to the class.
